Asthma

Asthma is a lung disease. Asthma causes the breathing tubes in the lungs to temporarily narrow. People with asthma have difficulty breathing air in and out of their lungs. An allergen or irritant in the environment usually triggers an asthma flare-up. There is no cure for asthma. Some asthma flare-ups can be prevented. Asthma can be treated with lifestyle changes and medication. Untreated asthma can become severe and life threatening. People can control their asthma with self-management and medical treatment. Most people with asthma can lead normal lives.

An allergy is the response of the body's immune system to normally harmless substances, such as pollens, foods, and house dust mite. Whilst in most people these substances (allergens) pose no problem, in allergic individuals their immune system identifies them as a threat and produces an inappropriate response. Coughing

What is Causing Your Allergy?

Allergic reactions are caused by substances in the environment known as allergens. Almost anything can be an allergen for someone. Proteins are organic substances which contain hydrogen, oxygen and nitrogen, and form an important part of all living organisms. There are also found in food, along with fats, carbohydrates and other substances. However, only proteins can cause true allergic reactions.

The most common causes of allergic reactions are

  • pollen from trees and grasses
  • proteins secreted from house dust mites
  • moulds
  • foods such as peanuts, tree nuts, milk and eggs
  • pets such as cats and dogs, and other furry or hairy animals such as horses, rabbits and guinea pigs
  • insects such as wasps and bees
  • medicines (these may cause reactions by binding to proteins in the blood, which then trigger the reaction)

Tuberculosis (TB)

India is the country with highest numbers of tuberculosis cases. Common symptoms of Tb are …fever, weight loss , cough more than 2 weeks. Commonly diagnosed with checking sputum for Acid Fast bacteria (AFB) but sometimes we take help of bronchscopy , EBUS and Medical Thoracoscopy to establish a firm diagnosis.

Sleep Disorder

Sleeping disorder as obstructive sleep apnoea are being diagnosed more and more. Common symptoms are day time somnolence, lethargy, chocking during sleep, irritability etc.

This disorder can be confirmed with Polysomnography and corrective measures can be suggested.

EBUS

  • EBUS is a relatively new tool on the block.
  • It is like doing bronchoscopy with a special scope with an inbuilt ultrasound.
  • It is used to sample lymph nodes in our chest to diagnose diseases like Tuberculosis, Sarcoidosis, lymphoma and malignancies.

Bronchoscopy

  • Bronchoscopy is endoscopy if breathing pipe.
  • It is done to look inside the airways and take sputum samples and small lung biopsies if required.
  • It can be done as an outpatient procedure.
  • Patient needs to be empty stomach for 4 hrs.
  • Blood thinners as clopitab should be stopped for 4 to 5 days if biopsies are to be taken.
  • It is fine under local Anaesthesia with sedation.
  • Patient should always come with an attendant as he cannot drive back himself.

Chronic Bronchitis

Endobronchial Ultrasound

It is a new modality and extention of normal bronchoscopy. Its advantage is that through it we can visualize the structures around main airways and take sample from them to make diagnosis.

Chronic Cough

Cough have several reasons and if cough stays for more than 2-3 weeks , it should be evaluated. Most common cause of chronic cough is Allergic Bronchitis but we need to rule out other serious reasons. So it is advisable to consult your doctor and go for basic work up.
